Regular Registration: Mar 15 - Jun 30

1. $150 Registration Fee
2. Payment plan available by paying $75 deposit upon initial registration and the balance of $75 by June 30. If full registration fee of $150 is not received by June 30 then the registration rolls into the terms of Late Registration period
3. $25 sibling discount applies for each child after the first fully paid child
4. In order to complete Regular Registration for $150 the following items are required by June 30 – anything less than completion of all of these items rolls the registration into Late Registration Period:
   a. $150 Registration fee less Early Registration discounts plus (if applicable) sibling discounts – all paid in full
   b. Completed and signed 2010 player/parent contract(s)
   c. Birth Certificate(s)
   d. Photo of child (2x2 including headshot)
   e. Complete 2009/10 report card
   f. Completed and physician signed 2010 Physical Form
4. Team registration is based on first come, first served basis – wait lists will be created as teams fill up
5. Discounted Youth Physicals Available: Visit Celebration or Hunter's Creek Americas Urgent Care for $25 Youth Physicals. Ask for the Pop Warner Rate. They should have copies of our physical form on file. Click Here for locations
